I understand that participation in the VBS program may include physical activities such as games, crafts, outdoor activities, sports, group activities, and other recreational or ministry-related activities.
I acknowledge that participation in these activities carries certain inherent risks, including but not limited to minor injuries, falls, physical exertion, allergic reactions, illness, or unforeseen accidents.
I certify that the participant is physically able to participate in the activities offered and that I have disclosed any medical conditions, allergies, medications, or special needs that may affect participation.
In the event of an emergency, I authorize VBS staff, volunteers, and/or emergency medical personnel to obtain medical treatment for the participant if I cannot be reached immediately.
I voluntarily permit the participant named above to attend and participate in the Grunthal VBS program organized by Pensy Chapel, Grunthal Abundant Life, CMC, Bergthaler and EBC churches in Grunthal.
To the fullest extent permitted by law, I acknowledge and accept all risks associated with participation in VBS activities and agree to release, waive, and hold harmless all churches and volunteers from any and all claims, liabilities, damages, injuries, losses, or expenses arising from participation in the VBS program, except in cases of gross negligence or willful misconduct.
